Jackson County Physical Therapy (JCPT) and the No Surprises Act
Under the No Surprises Act of 2022, all health care providers must provide a good faith estimate for services to anyone who is uninsured or self-pays (without insurance).
Good faith cost estimate of what you may be asked to pay if you are uninsured or out-of-network and opting to self-pay:
JCPT Cash Pay Rate Standard Services (all PT except TMJ, Pelvic Health): $125 per visit
JCPT Cash Pay Rate TMJ and Pelvic Health Services: $135 per visit
Questions about this notice and estimate? Call your JCPT office for specific questions.
Questions about your rights? Contact Oregon’s Division of Financial Regulation to speak with a consumer advocate via the following:

By signing Jackson County Physical Therapy’s financial policy, you are acknowledging that you have been provided with a good faith financial estimate of the costs of treatment with JCPT and that you agree to those terms. It is your right to pursue physical therapy services at the facility of your choice; if you are out-of-network with JCPT, you may contact your health plan to find in-network coverage for physical therapy services at another facility.
